Everwell Primary Care PLLC is committed to providing transparent and straightforward payment expectations for all patients.
Self-Pay Services
At this time, Everwell Primary Care primarily offers self-pay services. Payment is expected at the time of service unless otherwise stated in writing.
Accepted Payment Methods
Accepted forms of payment may include:

Insurance
Insurance credentialing is currently in progress. Patients are responsible for confirming whether services are covered by their health plan once insurance participation becomes available.
At this time, patients should expect to pay as self-pay/private-pay unless otherwise notified.
Payment Timing
Payment may be collected:

at the time of booking,

prior to the visit,

or at the time services are rendered.
Unpaid balances may need to be resolved before future appointments can be scheduled.
Fees and Charges
Fees for services may vary depending on visit type, complexity, and services provided.
Patients will be informed of self-pay pricing, when applicable, before or at the time of scheduling whenever possible.
Missed Appointment / Late Cancellation Fees
Missed appointments or late cancellations may be subject to a fee in accordance with Everwell Primary Care’s Cancellation Policy.
Non-Covered Services
Certain forms, letters, administrative requests, or other non-visit services may carry separate fees if applicable.
Refunds
Refunds, if issued, will be handled on a case-by-case basis and at the discretion of Everwell Primary Care, in accordance with payment processing policies and services already rendered.
